When you are ready to be vaccinated, please follow these instructions carefully:

Before Your Covid Vaccine Clinic Appointment

    1. Check your eligibility to receive the Covid-19 vaccination on the Department of Health Vaccine Eligibility Checker.
    2. The booking system has not yet been finalised – please do not call us to make a booking until this step has been updated. This step will be updated when we have more information. We anticipate to start vaccinating from the 8th of April 2021.
    3. Some clinics will start vaccinating from 22 March 2021. Like most clinics, we will only be receiving a limited supply of vaccines per week. The Covid-testing respiratory clinics will receive the majority of the vaccines. You can find a list of all currently active vaccination clinics via the Health Direct website. Our clinic will not appear on that list until April 2021.
    4. If you cannot demonstrate your eligibility when you arrive for your vaccination, you may be asked to leave. Aggression toward our staff will not be tolerated.
    5. If you have had another vaccination recently, including for seasonal influenza, you should wait at least 14 days before having your Covid-19 vaccination.
    6. Please read and complete the Covid-19 vaccination consent form BEFORE you arrive at the vaccination clinic.
    • You can print out the form at home, or pick up a copy from our clinic at Chevron Advanced Medical.
    • If you tick “Yes” to any of the questions, you must:
    • Call the National Covid-19 Vaccine Helpline on 1800 020 080.
    • Or, see your GP to check that you can still go ahead with the vaccination.
    • Or, book in to see one of our doctors at Chevron Advanced Medical to check that you can still go ahead with the vaccination.
    • Our reception staff are not clinicians and won’t be able to answer your medical questions.

    1. Please bring your Medicare card AND a photo ID.
    2. Please bring the completed Covid-19 vaccination consent form.
